LSC vacancies
To complete the nomination packet, you must fill out the Candidate Nomination Form, the Criminal Conviction Disclosure Form, and the Telephone Number Disclosure Form, with the Candidate Statement Form being optional. All forms are available in English and Spanish. You will also need to provide two forms of identification, which may include a driver’s license, state ID, voter registration card, Medicaid/MediPlan card, permanent resident card, matrícula consular, student ID, library card, lease, utility bill, credit or debit card, employment ID, or court documents.
