After-School Program Application

All extracurricular program sponsors must complete the following application in order to receive formal club recognition by John Hancock College Prep. After-school clubs cannot host any meetings until there is an approved application on file. All extracurricular club sponsors and students are expected to follow the policies and procedures outlined by Chicago Public Schools and John Hancock College Prep in order to maintain club status.
